Chronic Kidney Disease (CKD) is a condition in which the kidneys are damaged and cannot properly filter waste and excess fluids from the blood. It is a progressive disease, meaning it will worsen over time if left untreated. However, there are effective treatments available for CKD, especially if it is identified and managed early. The best specialists to help manage this condition are nephrologists.

Common causes of Chronic Kidney Disease (CKD) include diabetes, high blood pressure, kidney stones, and infections. Other contributing factors can be genetic disorders, immune system problems, thyroid disease, and kidney damage resulting from medications or exposure to toxins.

The symptoms of Chronic Kidney Disease (CKD) can vary depending on the stage of the disease. In its early stages, CKD rarely presents noticeable symptoms, and most people are unaware they have a kidney problem based on symptoms alone. However, as the disease progresses, common symptoms of advanced CKD may include fatigue, weakness, shortness of breath, ankle swelling, nausea, loss of appetite, and vomiting.

Chronic Kidney Disease (CKD) is diagnosed through a combination of medical history, physical examination, and laboratory tests. Blood tests are used to measure kidney function and blood salt levels, while urine tests help detect the presence of protein or blood in the urine. Additionally, imaging tests, such as ultrasound or CT scans, are performed to evaluate the structure and function of the kidneys.

Chronic Kidney Disease (CKD) is typically staged based on the level of kidney function, with Stage 1 being the mildest and Stage 5 the most severe. In Stage 1, there is kidney damage, but kidney function remains normal. Stage 2 involves kidney damage with mild impairment of kidney function, while Stage 3 indicates moderate impairment. In Stage 4, kidney damage leads to severe functional impairment. Stage 5 represents kidney failure, where the kidneys can no longer function adequately, requiring dialysis or a kidney transplant for survival.

Treatment for Chronic Kidney Disease (CKD) typically begins with tests to identify the underlying cause, followed by strategies aimed at preventing further decline in kidney function and managing symptoms. It is important to discuss your treatment plan with your kidney care team, as many therapies can be individualized. Treatment may include dietary changes, such as reducing sodium intake and following a balanced diet, as well as stopping tobacco use and pursuing weight loss if you are overweight. Medications are often prescribed to control blood pressure and blood sugar, and newer medications specifically target reducing protein leakage in the kidneys. Certain diseases, like vasculitis or immune-mediated conditions, may require specific medications to address the precise cause. Generally, a very high-protein diet should be avoided. While most people with a normal protein intake do not need to restrict it significantly, any drastic reduction should only be done after consulting with a nephrologist and renal dietitian. In advanced stages of CKD, fluid restriction may be necessary. Additionally, because CKD increases the risk of cardiovascular disease, treating high cholesterol is sometimes recommended. It is also important to avoid medications and herbal remedies that can affect the kidneys, and certain symptoms, like anemia, may require specific therapies.

In rare cases, CKD can be reversed. However, in most cases there are now good medications for stopping it getting worse if traeted early. Most people have a ‘reserve’ of kidney function and you only need a small part of your total kidney function to keep you out of kidney failure. Seeing a nephrologist early in CKD allows you and your doctor to plan your treatment to try to stop or reduce the rate of decline of your kidneys function.

The prognosis for people with CKD varies depending on the stage of the disease, the underlying cause and the treatment. In general, people with CKD are at increased risk for cardiovascular disease, kidney failure, and death. With early and modern treatment and management, many people with CKD have very good health and even stop progression of the disease. Even if the kidneys fail, there are many options in Abu Dhabi for transplantation and or dialysis therapy. Modern dialysis treatment is excellent and some can even be done at home. To explore options see the FAQ’s on Peritoneal dialysis, Haemodialysis and Kidney Transplantation.

Preventing Chronic Kidney Disease (CKD) involves taking proactive steps to reduce the risk of kidney damage and disease. Key prevention strategies include maintaining a healthy weight, eating a balanced diet, and reducing salt intake. Regular exercise is also important for overall kidney health. Monitoring your blood pressure and treating high blood pressure is crucial, as hypertension is a leading cause of CKD. If you have diabetes, managing your blood sugar levels effectively can help protect your kidneys. Additionally, avoiding medications and herbal remedies that can harm the kidneys is essential in reducing the risk of CKD.

A nephrologist is a medical doctor who specializes in the diagnosis and treatment of kidney diseases, including Chronic Kidney Disease (CKD). They play a crucial role in managing CKD by diagnosing and staging the disease, developing personalized treatment plans to manage symptoms, and slowing the progression of kidney damage. Nephrologists also monitor kidney function over time, adjusting treatments as needed, and provide education and support to patients and their families to help them better understand and manage the condition.

Diabetes can significantly impact kidney health, leading to a condition known as diabetic kidney disease (DKD). High blood sugar levels damage the tiny blood vessels in the kidneys, causing inflammation and scarring. This damage can result in the kidneys leaking protein into the urine, a condition called proteinuria, and can lead to a progressive loss of kidney function over time. If left unmanaged, DKD can advance to kidney failure, at which point dialysis or a kidney transplant becomes necessary to sustain life. Additionally, diabetes affects the heart and blood vessels, further contributing to kidney damage. It can also increase the risk of kidney stones, which cause pain and potential kidney injury, and raise the likelihood of infections, including urinary tract infections. The risk of developing DKD increases with the duration of diabetes, poor blood sugar control, high blood pressure, a family history of kidney disease, older age, and the presence of other kidney conditions. To reduce the risk of DKD, it is important to monitor and control blood sugar levels through medication and diet, manage blood pressure with appropriate treatments and a low-sodium diet, and maintain a healthy lifestyle that includes a balanced diet low in salt and sugar, regular exercise, and routine check-ups with a nephrologist. Taking prescribed medications consistently is also crucial. Early detection and treatment of DKD can slow the progression of the disease and reduce the risk of kidney failure.

A kidney biopsy is a medical procedure that involves removing a small sample of kidney tissue for examination under a microscope. It is usually perfromed as a day case medical procedure and rarely involves an overnight stay. If you have been asked to have a kidney biopsy your doctor may need to ascertain the type of kidney disease that you have in order to recommend the correct treatment, or define how likely it is that treatment will work.

SKC operates its own pharmacies at the three major kidney care centers, where all necessary medications can be obtained. For added convenience, these medications can be delivered directly to you during your hemodialysis sessions if needed. For out-patient medications, you have the option to obtain them from any SEHA or TLC pharmacy. Additionally, pharmacies offering delivery services, such as Dawak, are available for your convenience.

SKC does not directly provide transport services for patients, but they can assist in finding suitable transportation options. For ambulance services, SEHA ambulances can be booked for 1000 AED per trip. For standard car transport, SKC can help coordinate a car depending on the distance required.
